Modality Education Manager

As a Modality Education Manager, you will be responsible for leading a team of Patient Educators. Patient Educators specialize in new patient education, enabling patients to make an active and informed modality choice. They do this by teaching and educating patients and teammates on ESRD modality options including lifestyle benefits, challenges, techniques and alternatives. The educator is responsible for understanding the patient's values and adjusting the education to meet the needs of the patient, leveraging educational materials. The Educator Manager is responsible for the overall outcomes of each education program in their geography and for the readiness and impact of the educators on their team.

What to expect in this role:

Leadership and Support

Manage and supervise a team of field-based Educators, ensuring that they effectively and efficiently educate and inform patients, patient families, and teammates about treatment options and overall dialysis education and provide assistance with dialysis transitions as required;

  • Provide regular 1:1 observation of each Educator's patient interactions to ensure consistent and effective delivery of content (i.e., perform regular ride-alongs with each educator)
  • Coach Educators to collaborate with local teams to identify and educate potential and existing patients on specific modality options
  • Coach Educators to effectively coordinate and communicate with the clinic interdisciplinary team to plan education delivery and transition steps, when appropriate
  • Collaborate with local on-site leadership (including Regional Operations Directors, Home Division Leads, Facility Administrators and Regional Home Managers) to remove barriers to delivering education (i.e., scheduling, admit readiness, and team support/communication)
  • Adopt operational best practices developed and deployed by the Home National Team in partnership with Home Group Lead

Trend Analysis

Monitor, track trends and report key metrics key drivers.

  • Drive high productivity and effectiveness from each of the educators under their purview, ensuring program yields impact for patients. Ensure robust tracking and regular assessment of results
  • Regularly deliver feedback, assessment and training to Educators based on observation and metrics review, in partnership with Home National Team

Travel

  • Geography dependent, but up to 50% in the local region

Required Qualifications

  • Demonstrated ability to coach teammates (educators) on how to adapt messaging and education approach to the individual patient's circumstances and learning styles.
  • Experience providing direct observation, feedback, and assessment to teammates (or educators) in an education (or similar) environment.
  • Experience in project management to coordinate the effectiveness and efficiency of teammates (educators) while positively affecting patient experience.
  • Ability to cultivate connections with educators, peers, patients, leaders, and physicians.
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required.
  • A Bachelor's degree or equivalent experience.

Preferred Qualifications (a.k.a. nice-to-haves)

  • Minimum of one (1) year's nephrology experience, home dialysis experience.
  • Demonstrated knowledge of chronic, peritoneal, nocturnal, and home hemodialysis.
  • Experienced in case management, home health, disease management, or hospice.
  • Bilingual
