Position:Nurse Home Visitor

Shift: Exempt; FT Days

Primary Purpose:

Provides high-quality nursing care to patients in their homes. The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, which focuses on a particular patient population(s)/specific health condition, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assist patients in transitioning through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to healthcare personnel based on the Texas Nursing Practice Act, each patient's condition, and the competencies of the employee.

Job Summary:

  • The Nurse Family Partnership (NFP) and Nurse Home Visitor (NHV) is responsible for providing comprehensive community health nursing services to women and their families eligible for the NFP Program. The NHV is responsible for maintaining highest standards in clinical nursing practice and adherence to the NFP model, and to policies procedures, guidelines and standards of NFP and of Parkland Health and Hospital System. The NHV reports to the NFP Nurse Manager.
  • Develops and maintains community relationships to support client referrals. Conducts community outreach of potential program clients.
  • Develops client plan of care and communicates plan of care to clients and their families. Ensures implementation of plans to maximize quality and continuity of client care. Completes relevant records, intervening to optimize use of resources for cost effective care, utilizing a community-based approach.
  • Collaborates with all multidisciplinary team members and with a patient focus to implement the identified plan of care and facilitate and maximize client health care outcomes. Consults and collaborates with the Nurse Manager and the NFP team during case conferences in the interest of providing optimal client services.
  • Monitors the patients progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are client focused, high quality, and cost effective.
  • Facilitates postpartum/prenatal depression screening/counseling with clients and makes referrals for ongoing services as appropriate. 7. Maintains a positive relationship with physicians, nurses, other health providers, social workers, clients and their families to ensure that each client receives optimum care.
  • Uses a computerized database to verify and update information. Documents encounters with clients to ensure that assessment information is accurate and that each clients needs are served in a timely, efficient and cost effective manner.
  • Cooperates in review and analysis of the Efforts-to-Outcomes (ETO) reports for achievements and areas of improvement.
  • Identifies ways to improve work processes and improve customer satisfaction, serving as a client advocate to negotiate desirable client outcomes. Makes recommendations to manager, implements and monitors results as appropriate in support of the overall goals of the client. Interacts with other members of Nurse Family Partnership program team in ongoing program development, implementation and evaluation.
  • Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact nursing practice. Develops effective internal controls that promote adherence to applicable state/federal laws, and the program requirements of accreditation agencies and federal, state, and private health plans. Seeks advice and guidance as necessary to ensure proper understanding.
  • Stays informed on current health care developments to provide safe, quality nurse home visiting services. Completes all required NFP education. Actively engages in skill building to meet all NFP Home Visitor competency requirements. Stays abreast of the latest developments, advancements, and trends in the field of nursing by attending seminars, conferences, continuing education courses, reading professional journals, and participating in professional organizations. Maintains CPR certification and licensure as a nurse, with the Texas Board of Nurse Examiners. Integrates knowledge gained into current work practices.
  • Engages in special projects, and serves on committees, as assigned.

Minimum Qualifications:

  • BSN Degree from an accredited Professional School of Nursing.
  • Current/Valid State of TX RN license or valid temporary permit; or validCompact RN license in good standing without stipulations.
  • One (1) to two (2) year's direct registered nurse experience in maternal/child health, public health and/or home visitation.

Certification/Registration/Licensure:

  • Current/Valid Basic Life Support (BLS) for Healthcare Providers certification card upon hire and the ability to maintain thereafter from one of thefollowing:American Heart Association, American Red Cross, orMilitary Training Network.
  • Current/Valid State of TX Class C Driver License.
  • Current/Valid Automatic Background Check through TexasDepartment of Family and Protective Services upon hire and required completion every 2 years.
  • May be required to have additional specialty certifications.
