We are searching for a Registered Nurse - Supportive Care to perform a variety of duties within agency and department policies and procedures, assume responsibility for the primary care of assigned Hospice and Palliative Care patients and families. Assess, develop, implement and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending Physician's orders, Plan of Care and Hospice and Home Care policies and procedures. ***UP TO $15,000 NEW HIRE SIGN-ON BONUS*** (Details Below) Hours: 40hrs (full time) Shift: Day Shift 8am-4:30pm w/weekend & holiday rotations Travel: Will require driving
