We are searching for an LPN- Chronic Care Management. This role will be providing telephone Chronic Care Management for patients throughout the health system. Participate in the development of care plans to establish patient centered goals and the monitoring of outcomes. Drive engagement in comprehensive care management programs designated for patients with chronic conditions. Collaborate with providers and care teams to achieve goals and enhance patient's experience. Monitor and provide appropriate utilization management to achieve desired quality outcomes.



  • Hours: 40hrs
  • Shift: Day/Evening shift
  • Location: Primary Care - Fairhaven, MA
  • Travel: Driving on average 3 or more times per week required


Education and/or Experience Requirements:



  • Current Massachusetts or Rhode Island LPN license is required.
  • Minimum of 3 years of related job experience is required.
  • Experience as a Care/Case Manager or Coordinator either in the homecare, inpatient, physician practice or case management setting is highly desirable.
  • Prefer experience working as telecommuter or call center setting.
  • Experience in clinical telephone triage is preferred.
  • Experience working with Electronic Health Record (EHR) is required.
  • Demonstrated progressive proficiency with the utilization of available computer technology, including typing skills.
  • Demonstrated customer service, leadership, communication, interpersonal and problem solving skills are required.
