Job Overview:

The Social Worker MSW is a strong and collaborative team member who provides psychosocial patient assessments and supportive intervention, and develops plans for safe transitions from the hospital using an interdisciplinary approach. Manages complex social and discharge issues, builds and maintains relationships with internal/external resources, and links patients to community agencies and entitlement programs to address gaps in social determinants of health. Demonstrates an understanding of all stages of growth and development and the ability to assess each patient's age-specific and cultural needs to provide individual treatment and care.

Qualifications:

Required:

  • Master's of Social Work degree
  • NJ licensure (LSW or LCSW)
  • Minimally 1 year experience in an acute hospital setting (Will consider 1 full year of hospital internship)

Scheduling Requirements:

  • Day shift
  • Per Diem

Essential Functions:

  • Provides direct clinical social work services to patients and their families in an interdisciplinary team approach
  • Completing initial psycho-social assessments on assigned patients
  • Providing psychotherapeutic counseling with individuals, family or groups as needed
  • Providing patient centered advocacy, collaborating with an interdisciplinary team
  • Managing complex discharge plans, building and maintaining relationships with internal/external resources, making referrals to community agencies and entitlement programs and continuing ongoing education
  • Ensures continuity of care for patients with complex post-acute needs and maximizes the patients' and family's abilities to meet the continuing needs within their social and financial framework
  • Performs other tasks as directed
