PMEis looking for aHealthcareCase Manager I(RN, LCSW, LMSW) experienced in Case Management working with complex medical and mental illness cases. The position will require approximately field visits 2-3 days a week and the rest will be from home.Schedule M-F 8:30am-5:00pmCST. MUST live in within 30 minutes ofSouthside of Chicago (60617 & 60619) and surrounding parts. Responsible for health care management and coordination of healthcare members.Works with members to create and implement an integrated collaborative plan of care.Coordinates and monitors Healthcare members' progress and services.Provides case management services to members with chronic or complex conditions. Required Experience 0-2 years of clinical experience with case management experience. Required Licensure/Certification: No licensure required, LCSW, LCPC, RN are preferred. Must have Degree in social services or related field. Must have a valid driver's license with a good driving record and be able to drive locally if required.
